一、病理檢測(cè)
“病理”一詞相信大多數(shù)人都聽過,但又說不出個(gè)所以然。比如常規(guī)做的胃腸鏡檢查,如果檢查到有息肉組織,醫(yī)生會(huì)在切除息肉的同時(shí)送部分樣品到病理科做病理檢查,伴隨有一份病理診斷報(bào)告--這就是我們生活中接觸的病理組織學(xué)檢查及病理診斷。
“病理是診斷的金標(biāo)準(zhǔn)”,那到底什么是病理組織學(xué)檢查?對(duì)于病理診斷有怎樣的意義呢?
病理組織學(xué)檢查是指將通過手術(shù)或者內(nèi)鏡等操作后取下的身體某個(gè)部位的病變組織通過一系列的操作,做成病理切片,并經(jīng)過蘇木素-伊紅(HE)染色,亦或是免疫組織化學(xué)染色后供病理醫(yī)生在顯微鏡下觀察細(xì)胞形態(tài),來判斷組織發(fā)生的病變,是病理醫(yī)生進(jìn)行病理診斷判讀的重要依據(jù)之一。

二 免疫組化病理檢測(cè)
免疫組化,全稱是免疫組織化學(xué)(Immunohistochemistry,IHC),是病理診斷中一種重要的輔助檢查技術(shù)手段,主要根據(jù)抗原抗體反應(yīng)和化學(xué)顯色原理,組織切片中的抗原先和一抗結(jié)合,再利用一抗與標(biāo)記生物素、熒光素等二抗聚合物進(jìn)行反應(yīng),再用標(biāo)記辣根過氧化物酶(HRP)或堿性磷酸酶(AKP)等的抗生物素(如鏈霉親和素等)與酶標(biāo)二抗聚合物結(jié)合,最后通過呈色反應(yīng)或熒光來顯示細(xì)胞或組織中化學(xué)成分,在顯微鏡下可清晰看見組織細(xì)胞內(nèi)發(fā)生的抗原抗體反應(yīng)產(chǎn)物,從而能夠在組織切片上原位確定某些化學(xué)成分的分布和含量[2]。

免疫組化在臨床病理診斷中的意義[3]:
(1)惡性腫瘤的診斷與鑒別診斷;
(2)確定轉(zhuǎn)移性惡性腫瘤的原發(fā)部位;
(3)對(duì)某類腫瘤進(jìn)行進(jìn)一步的病理分型;
(4)確認(rèn)組織類型和種類;
(5)發(fā)現(xiàn)微小轉(zhuǎn)移灶;
(6)為臨床提供治療方案的選擇。
免疫組化在科研及新藥研發(fā)中的應(yīng)用:
研究疾病的發(fā)生機(jī)制:免疫組化技術(shù)可以用于研究特定蛋白在細(xì)胞和組織中的表達(dá)情況,從而為疾病的發(fā)病機(jī)制提供重要的參考。通過檢測(cè)不同疾病狀態(tài)下組織細(xì)胞內(nèi)抗原表達(dá)的變化,可以揭示疾病發(fā)生發(fā)展的分子機(jī)制。
輔助診斷和預(yù)后評(píng)估:在科研中,免疫組化技術(shù)可以幫助確定疾病的診斷,評(píng)估疾病的進(jìn)展和預(yù)后。
評(píng)估治療效果:免疫組化可以用于評(píng)估特定藥物或治療方法在組織或細(xì)胞中對(duì)目標(biāo)蛋白的作用。通過比較治療前后的免疫組化結(jié)果,可以衡量治療的療效,為個(gè)體化醫(yī)療提供指導(dǎo)。

PD-L1免疫檢查點(diǎn)抗體(Human PD-L1)

二 免疫組化病理檢測(cè)
免疫組化,全稱是免疫組織化學(xué)(Immunohistochemistry,IHC),是病理診斷中一種重要的輔助檢查技術(shù)手段,主要根據(jù)抗原抗體反應(yīng)和化學(xué)顯色原理,組織切片中的抗原先和一抗結(jié)合,再利用一抗與標(biāo)記生物素、熒光素等二抗聚合物進(jìn)行反應(yīng),再用標(biāo)記辣根過氧化物酶(HRP)或堿性磷酸酶(AKP)等的抗生物素(如鏈霉親和素等)與酶標(biāo)二抗聚合物結(jié)合,最后通過呈色反應(yīng)或熒光來顯示細(xì)胞或組織中化學(xué)成分,在顯微鏡下可清晰看見組織細(xì)胞內(nèi)發(fā)生的抗原抗體反應(yīng)產(chǎn)物,從而能夠在組織切片上原位確定某些化學(xué)成分的分布和含量[2]。

免疫組化在臨床病理診斷中的意義[3]:
(1)惡性腫瘤的診斷與鑒別診斷;
(2)確定轉(zhuǎn)移性惡性腫瘤的原發(fā)部位;
(3)對(duì)某類腫瘤進(jìn)行進(jìn)一步的病理分型;
(4)確認(rèn)組織類型和種類;
(5)發(fā)現(xiàn)微小轉(zhuǎn)移灶;
(6)為臨床提供治療方案的選擇。
免疫組化在科研及新藥研發(fā)中的應(yīng)用:
